Begrijp je nuancering inzake vergelijk ziekte-domeinen, dat was niet mijn intentie. Beter was geweest als ik dosis van GS-5745 binnen RA fase 1 had vermeld (GS-5745 400 mg administered intravenously**).

Goed om meer inzicht te hebben in kleine/grote monoklonale antilichamen, en dat molecuulgewicht het onderscheid bepaald.

De 400 mg toediening van Gilead duid dus op groot molecuulgewicht.
In Fase 1 RA kozen zij voor 1 infuus per 2 weken.

Is het dan niet vrij simpel door te stellen dat een combinatie-mogelijkheid is dat zij los van elkaar worden ingenomen/toegediend??, bijvoorbeeld zoals huidig bekende doseringsvormen:

(1) Filgotinib dagelijks toegediening (oraal)
(2) GS-5745 aderlijke injectie per bijvoorbeeld 2 weken.

Het is geen mix van 2 middelen, maar gaat los van elkaar en de weinige side effects van Filgotinib en goede JAK1 selectiviteit (Anemia beperkt en hemoglobin verbeterd) maken het enorm kansrijk voor deze combi.

Deze inhoudelijke toelichting wordt zeer gewaardeerd. (tevens @Rekyus, ABx2).

"De belangrijkste reden voor Gilead om in zee te gaan met Galapagos is omdat filgotinib zeer vermoedelijk de veiligste en meest effectieve aanvulling op GS-5745 biedt, omdat de anemia wordt beperkt en het hemoglobine niveau verbetert."

Dat zou inhouden dat filgotinib voor Gilead altijd al de "aanvuller" was. Is niet zo erg als het maar op waarde wordt geschat, en blijkbaar is men bij Galapagos erg content daarmee.

Gilead kan niet zonder een JAK1 Molecuul in dit stadium.
Filgotinib wordt leading in immunology portfolio Gilead met indicaties: RA en Crohn
In RA heeft Gilead nog niet veel bewezen.

Fase 1 GS-5745 is medio zomer 2015 completed. Vrijdag 25/9 zaten ze gelijk in het vliegtuig naar Belgie.
Filgotinib van Galapagos is unieke kans geweest voor Gilead.

Ze zullen zich volop gaan richten op immunologie therapien.

All starts with excellent molecules.

Als je het op juiste waarde wilt inschatten dan zou je moeten bedenken dat bij combinatie van GS-5745 en Filgotinib, het Galapagos-pilletje waarschijnlijk dagelijks moet worden ingenomen.
Ik zie de combinatie, net als Onno, dus als zeer positief.
Kortom als de combinatie superieur blijkt te zijn, zal de Filgotinib omzet dat ook zijn...

let wel op, ookal lijken het oogenschijnlijk verschillende MoAs, pas als je de 2 allebei tegelijk bestudeerd weet je dit zeker. aangezien vooral de JAK inhibitors erg nieuw zijn, kunnen deze nog wel eens wat side effects geven als het in combinatie vorm wordt toegediend. Als het echter een goed safety profile heeft, zal dit waarschijnlijk zeker een hogere efficacy hebben. hopen voor het beste dus, maar welke het ook wordt, filgotinib speelt een belangrijke rol hierin!

Staycalm, bedankt voor het delen.

Duidelijke Conference Call, en vooral uitgesproken verwachtingen omtrent CF en IPF; belangrijk om dit hieruit af te lezen.

CF houden ze alle opties open: voor nu C1, C2 en potentiator in paralel doorontwikkelen en eind 2016 keuze welke 3 moleculen als combo definitief de kliniek in ingaan.

Daarna snel doorpakken met start Fase 2 voor zomer 2017.
Achterstand op Vertex v.w.b. tijd zeer beperkt, zoals vorderingen nu zijn geschetst.

Daarbij heeft combo CF Galapagos in vitro zeer goede waarden laten zien (Consistend herstel HBE cellen > Chloride transport), waar Vertex met zijn verbeterde VX-661 (C1) hierbij achterbleef.

Met betrekking tot IPF goede kansen op markt: bij goede data Fase 2A mogelijk combined Fase 2B/3. Dit vernomen na vraag hierover; begin volgend jaar weten we meer.

Op naar FDA meeting; eerst RA (Maart start gesprek EoP2 meeting) en dan snel Crohn meeting (na 20-weeks scores van April).
Snel duidelijkheid geeft boost v.w.b. uitstekende veiligheidsprofiel Filgotinib molecuul.
Daarin verschilt het van de andere JAK's, evenals de voorziene brede inzetbaarheid van molecuul in meerdere ziekte-indicaties.

Nagevraagd bij Galapagos of bij opstarten Combi studie CF-patienten ook Azië wordt aangedaan.

volgens mij gaat abbvie de phase 3 uitvoeren, maar met vergoeding (gedeeltelijk) van glpg, eigenlijk een beetje het zelfde als gilead gaat doen met filgo. Denk dat glpg die fase 3 trials liever nu nog aan wat grotere bedrijven geeft, die het bij wijze van spreke '' voor kunnen doen''. Wanneer we een aantal jaren verder zijn zullen ze wellicht dit zelfstandig aan willen pakken. Ik vind het best een prima manier, waarbij je toch veel leert en veel contribute, maar een ervaren partij naast je hebt staan

Let's switch gears a little bit to go to Filgotinib, but I know there is a lot of - obviously a lot of the discussion over the past couple of years has centered around this dev, but is this the kind of opportunity that Gilead should that we should expect from Gilead when it comes to sort of wholly-owned, really bigger therapeutic markets opportunity for differentiation. And is this - I guess what perspective can you give us about what you're thinking about as new CEO versus John Martin.

John Milligan

Well, so John and I worked together for 26 years. So, first of all, we'd expect anything dramatically different out of what ongoing versus what we were doing together. And of course, he's still Executive Chairman of the company.

So, I can't pay to anyone particular bucket, but this is the kind of thing we're going to do. When you're a growing company like Gilead is, when we have ambitions to go in different areas. You have to find the right deal at the right time. And so Filgotinib was a very interesting opportunity. We had behind-the-scenes been exploring a number of different compounds that we have for inflammatory diseases and had sort of quietly build up a team of experts in this area. Some of it centered around our MMP9 antibody called 5745, which by the way in phase 2 studies has shown activity in ulcerative colitis RA and is being tested and things like COPD.

In addition to syndications in cancer and that's because MMP9, the target of the antibody is a highly over-expressed in a lot of these different diseases and so it's associated with them and when we tested our MMP9 antibody in fact it has a very pronounced effect. So, we were starting to explore and think about, okay, could we build a franchise around MMP9 or could MMP9 be a part of a franchise that would involve oral medications. And of course we have a lot of oral medications that knocked down B-cell functions such as our BTK, our Syk inhibitor and our PI3 kinase delta inhibitor. The question is would we have anyone of those have the right activity.

As this was occurring, we were also doing combination studies with our JAK inhibitors, with various JAK inhibitor with our own portfolio and found an interesting synergy or at least additivity across many of them. So when Filgotinib became available, it was something, it was quite interesting to us. And in thinking about the right way to make sure that we got access to a JAK inhibitor, it became clear to me that the right way is to form a partnership with Galapagos except with what they were seeking that was the right way for them to further their causes as an independent company and essentially gave us worldwide rights to the product, which is something that we saw it. And so, I think it's a very nice partnership and an example of where you have to be flexible to get the product you want, under the terms you want for the right parts of the world and so it worked out very nicely for both companies.So that's not to say, so I think your question that we're looking for big markets. We're looking for markets big and small. We're looking for opportunities where there are scientific advancements to be made. I think Filgotinib have some really nice attributes that can be used in a wider range of diseases then are currently being explored. So it's a very interesting opportunity for us to see if we can build something that will be meaningful in the not too distant future. And its phase 3 ready. So it will start our phase 3 studies in the third quarter of this year. I want to caveat that but we still have to have some conversations with FDA about dosing and final protocol. But from my point of view of the risk benefit to go to higher doses clearly there and we hope the FDA will agree to that.

Ik lees hierin ook de eis van Onno om zelfstandig te blijven waar Gilead maar aan moest voldoen omdat ze waarschijnlijk anders naar een concurrent waren gestapt

Was er al eerder gesproken over start q3 ipv h1? 494 loopt daarmee weer een extra kwartaal uit. Trial design wordt interessant, en ben benieuwd hoe snel patiënt werving zal verlopen. Een mooie flyer met Darwin 3 testimonials kon de recruiting nog wel eens flink helpen.

Verder ook hier weer nadruk op de combi therapie. Ben benieuwd hoe spoedig we met ph2 combi trails worden verblijd.
